For self-testing. For use with Contour Nest gen. Contour next, Contour next EZ, Contour next USB, Contour Nest one, Contour Next link and Contour Next link 2.4 meters. Important: For in vitro diagnostic use. Before using this product, read the package insert carefully. Use of this device on multiple patients may lead to transmission of Human Immunodeficiency Virus (HIV), Hepatitis C Virus (HCV), Hepatitis B Virus (HBV), or other bloodborne pathogens.
Store between 41 degrees F-86 degrees F and 10%-80% relative humidity.

Ingredients
Reagents: 21% w/w FAD Glucose Dehydrogenase (Aspergillus sp., 4.0 U/test strip). 54% w/w Mediator. 25% w/w nonreactive ingredients.
